CTC-RI Annual Conference:“Advancing Primary Care:Innovating for a Sustainable Future”

Target Audience
All CTC-RI practices, learning collaborative participants and partners, past and present, are welcome to attend. We invite practice leadership, providers, nurses, nurse care managers, care coordinators, behavioral health staff, practice managers, pharmacists, medical assistants, community health workers, and CTC-RI stakeholders to come learn as a team!
Registration Information
Registration will begin at 8:00am EST with a light breakfast. Our welcome address will begin at 9:00am.
CME Accreditation
CME’s/CEU’s and certificates of attendance will be available after the conference. CHW certificates of attendance will also be available.
